Application Forms for ASSO CET 2012 by AMUPMDC

ASSO CET 2012 is an entrance exam conducted by the Association of Management of Unaided Private Medical and Dental Colleges, Maharashtra (AMUPMDC) for admission to various medical and dental courses in unaided private medical and dental colleges in Maharashtra. ASSO CET 2012 will take place on 5th May 2012.

Here are the different ways in which you can get ASSO CET 2012 application forms:

1. Prescribed application forms can be obtained from the notified centres against DD drawn on the nationalized banks in the name of the "AMUPMDC" and payable at "Mumbai" for Rs. 500/- (Rupees Five Hundred Only). (Click here to see the list of notified centres selling ASSO CET 2012 Forms)
2. Application forms will also be available from the Association’s office by post with an additional DD of Rs. 200/- for postal charges:

3. Forms will also be available from selected branches of Punjab National Bank (PNB) on cash payment of Rs. 500. 

How to Apply for ASSO CET 2012

a. The ASSO CET 2012 application form must be filled in your own handwriting using only black ballpoint pen. The application form complete in all respect shall be submitted directly to the office of
AMUPMDC,
Shrijee House, 75 Mint Road,
Mumbai 01
either in person or by speed post only. No private courier will be accepted.

b. The filled-in application form is to be duly returned in the envelope provided by AMUPMDC with the following documents:

• Filled-in Application Form
• DD for Rs. 1,500/- (Rs One thousand Five Hundred only) drawn in favour of "AMUPMDC" and payable at Mumbai (for Asso-CET 2012 Examination Fee).
• Receipt-Cum-Identity Card,
• Self-addressed envelope with appropriate postal stamp (for sending back the receipt)

c. The application form should be submitted well before the last date.

d. AMUPMDC will not be responsible for postal delay.

e. Application forms submitted by speed post MUST be accompanied by a self-addressed and stamped envelope along with the Receipt-cum-Identity card for acknowledgement. Incomplete application forms will be rejected.

f. The official at the AMUPMDC office while accepting the application form submitted in person shall issue the “Receipt cum Identity” card attesting your photograph with signature and stamp.
