Application Form for MGIMS PMT 2012: MBBS Entrance Exam

The Mahatma Gandhi Institute of Medical Sciences invites applications for admissions to its MBBS course starting from 1st July 2012.

The Prospectus and Application Form for MGIMS PMT 2012 along with the syllabus for the MBBS entrance exam, the question papers of Gandhian thought for the last 5 years, with model question papers of Physics,Chemistry & Biology can be obtained from the office of the Dean either by -

1. Paying a sum of Rs. 1000 personally at the cash counter of Dean’s Office.

2. Remitting a sum of Rs 1100 (Rs.1000 + Rs.100 as postal charges) at any branch of State Bank of India through a Bank Challan.

• If you want to submit the fee by bank challan, click here to access the bank challan. Fill the information required and submit it.
• You should mention your full name, address with pin code number on which you want to receive the prospectus & phone number for communication in the space provided on the challan form so that the prospectus can be sent to the given address.
• You should then take a printout of the saved copy and visit the nearest branch of State Bank of India for submission of the required charges.
• The Challan form is in 3 copies and you have to send 1 original paid copy of the challan form to the “Dean, Mahatma Gandhi Institute of Medical Sciences, Sevagram, 442102” by speed post.
• The prospectus will be sent to you on receipt of the copy of the challan stamped paid by the bank.
• The Institute will not be responsible for delay/loss in transit of application form due to postal problem or incorrect address without pin code provided on the challan form by the applicant.
• The cost of application form is same for all the candidates
